Quality Improvement

  

Maple Family Health Team is dedicated to improving the quality of primary care services for you and your loved ones.  We constantly evaluate the services we provide as we strive to enhance the patient experience and improve health outcomes.  Maple has a Quality Improvement (QI) Committee, made up of physicians, interdisciplinary health providers, and administrative staff.  Our quality improvement initiatives are developed with consideration for the priorities for primary care which are set by Health Quality Ontario.  Quality Improvement is a province-wide effort.  Health Quality Ontario (HQO) supports and promotes sustainable improvements in care across Ontario.

 

Quality Improvement Plan (QIP)

Maple Family Health Team submits a Quality Improvement Plan annually, which outlines
our projects for the year, as required by the Excellent Care For All Act, 2010.

This plan enables us to review our targeted areas for improvement and chart our progress to improve
the services we provide, to deliver the highest quality health care.

We created a Patient and Family Advisory Council for a more patient-centered approach.  For further information, please visit here.

Maple's pharmacist completed medication reviews for our patients with complex conditions within 14 days of being discharged from the hospital.

We contacted our patients who were eligible and overdue for colorectal screening.  We made a significant stride in the prevention and early detection of colorectal cancer.
