We value your feedback as it helps us to improve our care and services. You may provide us with comments, concerns and suggestions by mail, email or by completing the form below.

  • When you wish to share ideas for making services better.
  • When you have concerns about your rights or privacy.
  • When you wish to send thanks to a Maple employee for providing excellent care.
  • When you would like to provide us with feedback about our accommodations for people with disabilities.

This process is intended to be for general comments and suggestions about Maple's services, and should not pertain to your own healthcare.  If you have questions about your health, please contact your clinic directly.
