Our Patient and Family Advisory Council (PFAC) is here to help us improve the patient experience at Maple Family Health Team.
The Maple Family Health Team (Maple) is a primary health care organization that brings together family doctors, nurse practitioners, registered nurses, social workers, dietitians, and other professionals to provide and coordinate the best possible primary care for patients. Maple provides team-based care throughout six clinics in the Kingston area.
This council works in an advisory scope, making recommendations on matters that relate to patients, families, and the primary healthcare experience. Are you interested in sharing feedback to help improve quality of care?
