We’re excited to share that Maple Family Health Team is joining together with Loyalist Family Health Team to form one stronger, more connected organization.
The amalgamated organization will continue as Maple Family Health Team.

What does this mean for you?

Right now - very little will change. You will continue to receive your care through Maple, with the same team you know and trust. Your clinic location and after-hours care will remain the same.

Over the coming months, we’ll take a thoughtful, gradual approach to bringing our teams and services together. Our focus is on learning from both organizations and building on what works best, so we can continue to provide high-quality, patient-centered care.
