Maple Family Health Team

At the Maple Family Health Team (FHT), we offer a variety of services and programs to meet the needs of our patients. In addition to the care that you receive from your family doctor, the Maple FHT also has other health care providers available to support your needs. These additional programs and services are listed below and are often provided right in your doctor’s office. These programs support your health, by either helping to manage an existing disease or assisting in preventing future disease.

General Medical Care

The family doctors of the Maple Family Health Organization provide the majority of your medical primary care.  The Maple Family Health Team is complementing these services with nurse practitioners and other health care providers. Our Nurse Practitioners work in collaboration with Physicians and Registered Nurses to care for patients in many different ways. 

Some of your care will be provided by other health care providers within the team either by referral or as part of general medical care. Each of the programs listed describes how other health care providers support your care.

Stress Management
Many people feel the pressures of everyday life. Stress can impact health and well-being. Maple FHT has developed a Stress Management group program. The program is designed to help participants understand the impacts of stress on their health and learn ways to manage the stress in their life. This program is led by a Mental Health Counsellor.

Introduction to Stress Management

Participation in this program begins with attending a 1-hour “Introduction to Stress Management” session. At that session, participants learn about sources of stress and the link between chronic stress and disease. They are introduced to some ways to manage stress in their lives. The “Introduction to Stress Management” is a prerequisite for attending future modules on specific Stress Management topics.

Space is limited in this group so registration is required. Patients interested in attending an Introduction to Stress Management session can contact their doctor’s office to register.

Stress Management Modules

After attending an Introduction to Stress Management, participants are eligible to participate in one or all of the Stress Management Modules that will be offered after the program. Dates and times for the modules will be communicated at the Introduction session.

Topics covered in the Stress Management modules include such things as:
  • Progressive Muscular Relaxation
  • Dealing with worry and anxiety
  • Time Management
  • Assertiveness
  • Work-Stress management
  • Exercise
  • Sleep Hygiene
Weight Management

This is a non-dieting, healthy lifestyle approach designed to help overweight and obese patients improve their eating habits and find ways to increase activity levels. This group program is offered to help participants:

  • Achieve a healthier weight
  • Reduce their health risk
  • Feel better both physially and mentally
  • Improve their nutrition status
The group is led by a Registered Dietitian, with components facilitated by a certified Personal Trainer and a Mental Health Counsellor.  Group participants are Maple FHT patients who have previously attended an "Best Weight Introduction" group.

Chronic Obstructive Pulmonary Disease Management

Diabetes Management

The Maple Family Health Team (FHT) “Diabetes Management” program has been developed to improve the patient’s health and to assist patients in having a controlled and managed disease. The Maple FHT nurses are trained in the diabetes Clinical Practice Guidelines and work collaboratively with your family physician or Nurse Practitioner. The program is developed around regularly scheduled visits where nurses review and record the recommended aspects of a diabetes patient’s health. These visits may also include patient education in areas such as: glucose control, meter use, hypoglycemia, taking medications, healthy eating, foot care, and more. This regular monitoring, in conjunction with the care provided by your family physician, supports patients in managing their diabetes.

Flu Shots




Regular immunization is an ongoing part of patient care at the Maple Family Health Team. Immunization against disease helps in the prevention of disease and most immunizations can be performed in your physician’s office. You or your family can also get immunized at your local Public Health Unit. Information regarding immunizations is available through the Public Health Unit. 

Mental Health

Many people will experience a mental health-related problem or mental illness at some point in their lives. The Maple Family Health Team (FHT) has a team of Mental Health service providers to assist people at these times. The Mental Health team consists of a psychiatrist, psychologist and mental health counsellors. Family doctors and the Mental Health team work together to help people with mental health related problems.  These Mental Health Services are available to people of all ages, including children, teens, adults and seniors. The Maple FHT continues to build links to other Mental Health service providers in the community to provide you the best care available.

Clinical Nutrition Services

 Registered Dietitians (RD's) are experts in the area of nutrition as well as interpersonal and counselling skills.  They are trained to provide individualized, evidence-based nutrition therapy to patients.  The long term goals are to optimise nutrition status, improve health, increase longevity and enhance enjoyment of life.
